Care Plans for Chronic Disease Management

Care-PlanOur doctors are able to put a GP Management Plan or Care Plan in place for patients with chronic conditions such as asthma, diabetes, heart disease, arthritis, and other conditions.

A Care Plan acts as a central document where we can keep all the information about how to best manage your condition, as well as which other health providers are involved in your care.

One of the great things about having a Care Plan is that it may give you access to up to 5 sessions with an allied health service provider such as a physiotherapist, podiatrist, dietitian, exercise physiologist, chiropractor, or osteopath, which are partly covered for by Medicare.

Getting a Care Plan also gives you a great opportunity to be more actively involved in the decisions around what treatments and providers you would prefer, and what goals you would like to achieve for your long-term health.

To be eligible for a Care Plan you must have a chronic condition, such as asthma or diabetes for example, and you must book an appointment with our nurse who will guide you through the plan. You can then discuss what services and providers may be most suitable to meet your needs.
